Employee Basic Life

  • 2x annual earnings (excluding bonus and OT)

  • Max: $1,500,000*

*Medical underwriting required for amounts over $1,000,000

Spouse Basic Life

  • $5,0000 benefit for spouse

Child Basic Life

  • $2,000 benefit

  • Unmarried child dependents up to age 26

Supplemental Life Insurance

Employee Supplemental Life

Purchase in increments of $10,000 up to $1,000,000 not to exceed 5 times the employee base salary*

*Evidence of Insurability (EOI) may be required

Spouse Supplemental Life

Purchase in increments of $5,000 to $250,000 but not to exceed 50% of Employee Supplemental Coverage*

*Evidence of Insurability (EOI) may be required

Child Supplemental Life

Purchase in increments of $2,000 to $10,000 but not to exceed 50% of Employee Supplemental Coverage*

*No evidence of insurability required

Do I need to complete an Evidence of Insurability (EOI) when electing supplemental life?

Employee Supplemental Life

You will need to complete an EOI, Evidence of Insurability, if you answered YES to any of these questions:

  • I am not currently enrolled in supplemental life, but am choosing to enroll for 2021

  • I am increasing my supplemental life coverage by more than $50,000

  • I am increasing my supplemental life coverage over $300,000

Note: New hires in their new hire enrollment will not have to complete an EOI for employee supplemental coverage under $300,000

Spouse Supplmental Life

Your spouse will need to complete an EOI, medical certification, if you answered YES to any of these questions:

  • My spouse is not currently covered under Gerber's supplemental life, but I am choosing to enroll for 2021

  • I am increasing supplemental coverage over $25,000.



Note: New hires in their new hire enrollment will not have to complete an EOI for supplemental spouse coverage under $25,000

EOI FORM

If you are required to complete an EOI based on the requirements listed above, you must submit this form and send it to Prudential directly . Coverage will not be effective until the EOI form is sent to and approved by Prudential.

Accidental Death and Dismemberment (AD&D) Insurance

Basic AD&D

  • 100% Gerber Paid

  • 2x annual earnings (excluding bonus and OT)

  • Max: $1,500,000*

*Medical underwriting required for amounts over $1,000,000

Supplemental AD&D

  • Purchased in increments of $10,000 up to $1,000,000 for you or your family.

*Medical underwriting not required

Short Term Disability (STD)

100% paid by Gerber

Benefit Amount: 100% of weekly earnings for the first 4 weeks and then 75% of weekly earnings for weeks 5-26 minus other income benefits up to $2,500

Waiting Period: 0 days for accident; 7 days for illness

Duration: Up to 26 weeks

Benefit Amount: 60% of monthly earnings up to $20,000 max.
